Write a legible handwritten letter containing your full legal name, date of birth, residential mailing address and registered account email. Include a self-addressed stamped return envelope, standard size #10 or larger, under the proposed instructions.
Use one request per outer envelope. No GC purchase is required. Postage costs may apply. Eligibility and authenticity may be checked.
